Crime Legal Aid Form
Please click on the links below to download and complete the forms. We will only be acting for you once we have accepted your case. Completing the forms does not constitute us accepting instructions. Guidance for completing CRM14 and CRM15.
Along with completing the above forms, you will need to provide us with at least the following:
- 3 months of wage slips
- 3 months of bank statements
- Tenancy agreement if you are not living with parents. If you are living with parents and paying them rent we would need proof of this
- Council tax payments (if you are making any)
- If you have other sources of income please provide all supporting documents such as companies, self-employed income – see the explanatory guidance for applicants.
If you or partner are on benefits please provide us with your national insurance numbers and the benefits which you are receiving.
If you or your partner are not working and not in receipt of benefits please let us know how you are supporting yourself financially (living with family, living with friends, homeless).
